      目的 探讨术前C反应蛋白(CRP)值对重症创伤骨折患者术后并发下肢深静脉血栓的影响。方法 回顾分析329例重症创伤骨折患者的临床资料,确定CRP最佳临界值,分析在众多可能影响重症创伤骨折患者术后并发下肢深静脉血栓的相关因素中(年龄、吸烟史、高血压、糖尿病、体重指数、冠心病、是否开放性损伤及CPR值)CRP是否为独立影响因素。结果 确定CRP的最佳临界值为33.95 mg/L,将患者分为高CRP组(CRP≥33.95 mg/L)与低CPR组(CRP<33.95 mg/L)。经单因素分析,年龄、糖尿病、冠心病、体重指数及CRP是重症创伤骨折患者术后并发下肢深静脉血栓的影响因素,进一步行多因素Logistic回归分析,年龄、糖尿病、冠心病、体重指数及CRP均是影响重症创伤骨折患者术后并发下肢深静脉血栓的独立因素。结论 术前CRP值是重症创伤骨折患者术后并发下肢深静脉血栓的危险因素,对重症创伤骨折患者术后是否容易发生下肢深静脉血栓具有一定的预测作用。

      ObjectiveTo investigate the relationship of preoperative C-reactive Protein (CRP) and the complication of deep vein thrombosis in patients with severe traumatic fracture.MethodsThe clinical data of 329 severe traumatic fracture patients were retrospectively analyzed to determine the optimal critical value of CRP. We analyzed whether CRP was an independent factor affecting the complication of deep vein thrombosis after the severe traumatic fracture surgery. (The factors that affect the recurrence include age, history of smoking, hypertension, diabetes mellitus, body mass index, coronary heart disease, open or closed injury and CPR value.) We analyzed whether CRP was an independent factor.ResultsWe determined the optimal critical value of CRP was 33.95 mg/L. We divided the cases into the high CRP group (CRP≥33.95 mg/L) and the low CRP group (CRP<33.95 mg/L). Single factor and multivariate analysis indicated that age, diabetes mellitus, body mass index, coronary heart disease, and CPR were the independent factors affecting the complication of deep vein thrombosis after the severe traumatic fracture surgery.ConclusionPreoperative CRP is a risk factor for deep vein thrombosis after the severe traumatic fracture surgery and has important predictive value on the complication of deep vein thrombosis after the severe traumatic fracture surgery.
